Community Reentry Project (CRP) is a multi-stakeholder project that coordinates efforts to ensure that people living with and at the highest risk for HIV transition smoothly from incarceration to communities. CRP facilitates a comprehensive system of HIV prevention, care, and support services throughout Illinois, and provides technical assistance and capacity-building support to agencies serving the corrections and reentry populations. Through CRP, PHIMC focuses efforts to develop a public health response to systemic societal barriers impacting successful reentry into Chicago and Cook County communities for formerly incarcerated individuals living with or at risk of contracting HIV.

CRP activities include Illinois Reentry Statewide Meetings, Chicago Reentry Taskforce Meetings, Reentry Support Trainings, an annual Illinois Reentry Conference, and Summits of Hope. PHIMC also supports partner organizations with technical assistance and strategic planning.
