Community Reentry Project (CRP) is a multiple-stakeholder initiative that coordinates reentry efforts in Illinois. CRP focuses on ensuring that people living with and at the highest risk for HIV transition smoothly from incarceration to communities. These individuals need a comprehensive system of HIV prevention, care, and support services to accommodate this goal. PHIMC serves as the convener and grants manager for the six partner agencies, guiding strategic planning and providing fiscal management, program management and monitoring, and grant reporting.
